Cooperation between pharmacies and medical services at the hospital and its impact on therapeutic management

This intervention-research concerns the management of medicines on the site of a CHU (University Hospital Center) and the analysis of the organizational dysfunctions, as well as the setting up and evaluation of change processes, co-elaborated by its pharmacy and its medical services, in order to reach a better performance in the process dealing with medicines and, finally, a better safety for patients. A preliminary phase for listening to actors, followed by a system aiming at co-elaborating improvement solutions has permitted to mobilize actors in the pharmacy and the medical services. The actions implemented and the indicators, have partially improved cooperation. Beyond that, it’s the place of pharmacies and pharmacists which is challenged.
