Matters of justice and equity in transplantation medicine constitute a very difficult task for the medical profession as well as for philosophers and social scientists. In this context it can be helpful to review the different levels where allocation problems occur, the longstanding discussion on discrimination concerning access to transplantable organs, and the different principles that can be employed for their distribution. An awareness of the current legal regulations as well as their problematic aspects will will contribute to advance the search for the best possible solution, on a general level as well as for each individual patient.
