The early treatment of acute ischemic stroke has been
significantly modified since the publication of the first positive trial in 1995 (NINDS). Subsequently there has been an
explosion in acute treatment modalities and its administration as well as the profile of the patients accepted for the
treatment. Simultaneously, a better organization of the
stroke networks allowed to provide treatment for a larger
number of patients. The following chapter gives a review of
the thrombolysis trials and summarizes the inclusion and
exclusion criteria of intravenous and intra-arterial use of
the pharmacologic and mechanical thrombolysis. It also
presents the new pharmacotherapies and mechanical
devices invented to expand the treatment window and
increase the safety and effectiveness of the thrombolysis in
acute ischemic stroke.
