Giant Syphilitic aneurysm of internal carotid artery with fatal prognosis by cataclysmic rupture in a young man infected by HIV. A case report

Abstract

Authors report a rare case of giant carotid syphilitic
aneurysm with surrounding structures compression. It is a
58 year-old patient with a history of giant ulcer 10 years
ago without other cardiovascular risk factors apart from
age, which was referred for the management of a neck
right size painful mass. Examination revealed a large right
size neck mass, 22 cm of major axis associated with
hoarseness and difficulty breathing of compressive origin.
The cervical CT angiography confirmed the diagnosis of
right internal carotid aneurysm with compression of the
right external carotid artery and biology found a positive
VDRL and TPHA serology. During transfer to surgery, a
rupture with cataclysmic external bleeding occurred and
patient died.
